Job Title: Care Coordinator Specialist
About the Role:

Key Responsibilities:
* Coordinate nurses and care workers to ensure efficient allocation of care visits.
* Monitor and review care plans, addressing day-to-day issues raised by service users.
* Handle enquiries from nurses and carers, escalating when necessary.
* Use Careplanner to fill unassigned shifts promptly and effectively.
* Ensure no shift goes unfilled, possibly attending shifts at short notice.

Requirements:
* Level 2 in Health and Social Care or above / or Proven competence in the role and willing to undertake qualifications.
* Commitment to working unsociable hours when necessary.

Promoting business development opportunities and undertaking tasks as directed by the Registered Manager.
A commitment to working 24/7, participating in the 'on-call' rota, providing emergency support as needed.
