Overview
Job summary: The successful candidate will work alongside existing members of a supportive Community Palliative Care Team (CPCT) and will contribute to the delivery of palliative care by providing expertise in decision-making, advance care planning & Holistic symptom control. This position will be within the North Wiltshire area.
Main duties
To work with the CPCT Clinical Lead to develop a program of continuous quality improvement and service development to improve patient, family, carer and staff experience.
