Overview

Join to apply for the Support Time Recovery Worker role at Kent and Medway NHS & Social Care Partnership Trust.

Note: At this stage the role is open only to colleagues currently employed within Kent and Medway NHS Trusts and the Kent and Medway Integrated Care Board (ICB).


Responsibilities

* Support service users by delivering Dialog+ and assisting with interventions.
* Assess and plan with service users in collaboration with the Clinical Pathway Leads.
* Deliver group-based interventions and promote health, safety, and best practice.
* Contribute to assessing clients referred to Mental Health Together or Mental Health Together Plus.
* Work with individuals to facilitate access and engagement in leisure and community-based activities, across care boundaries within Mental Health Together or Mental Health Together Plus.
* Assist in delivering high-standard support to service users and carers, promoting equality, dignity and mental well-being.
* Actively listen to help service users understand their mental health problems using a basic CBT formulation model and help develop coping strategies based on CBT and mindfulness approaches.


Mental Health Together + and Model

In Kent and Medway, Mental Health Together is a community mental health transformation and a multi‑agency offer that brings services together to provide the right care without navigating service boundaries or retelling their story.

Role: Mental Health Together will introduce new roles as part of a joined‑up model of care bridging primary care and community mental health to treat people with complex needs. It includes:

* Providing person-centred, holistic support
* Removing barriers to access and thresholds across services
* Involving service users and carers as equal partners in care
* Building involvement and co‑production in service design and delivery
* Delivering needs-led and personalised trauma‑informed care
