Qualified Social Worker, Hospital TeamPay rates to £30 per hour Contract roleVitalis are recruiting for an experienced Socia Worker to work in in a hospital team in Huddersfield.The role requires the social workers to assess service users in hospital under the Care Act and to co-ordinate a care and support plan to support a timely discharge from the acute hospital. The social worker will be expected to carry out Mental Capacity Act assessments and Best Interests decision where appropriate. There is an expectation that reviews will need to be undertaken following the hospital discharge including the need to attend Decision Support Tool meetings. Adult Safeguarding work may also have to be undertaken as part of the role. We do have a duty rota and after gaining further experience in the team the agency social worker would be expected to cover duty work.The role is full time - 37 hours a week Mondays to Fridays.Work with Vitalis to Discover Exciting Opportunities in Qualified Social Work, Housing and SEND
