Specialist Community Public Health Nurse Trainee ( School Nurse)

If so, Livewell Southwest has an exciting opportunity for you to join the Public Health Nursing service as a trainee School Nurse ( 18 month fixed-term, full time hours).
The successful candidates will work within the locality PHN teams across Plymouth. The competency-based framework and learning opportunities will support your access to the Specialist Public Health Nursing Programme (SCPHN/SN). This post will be supported by the Team Managers, Practice Assessor and Colleagues.
The specialist Public Health Nursing programme is offered by Livewell Southwest and provider of the academic programme is to be confirmed.
This role may not be eligible for sponsorship under the Skilled Worker route; please refer to the Direct Gov website for more information with regards to eligibility.

You will be working as part of an integrated skill mix team, supporting the implementation of the Healthy Child Programme under the direction of the School Nurses. Delivering the service requirements, you will be working in a variety of community settings, across one of the five localities, including homes of families. We ask that you have excellent interpersonal skills, the ability to work in partnership with other teams and agencies, along with great communication skills and a passion for working with children young people and families.

The teams current operating hours are Monday to Friday, 9am to 5pm.

To work with individuals,families, schools and the community
To work in multi-agency partnerships, for example, SocialCare, Education, Police and Voluntary Services.
To work inter agency with 0-5yrs Health Visiting, Speechand Language and Primary Care Teams.
To work in liaison with University Hospital Plymouth NHSTrust departments, e g Paediatric Outreach, Emergency Accident, Child andAdolescent Mental Health Service.
To work with Educational establishments e.g. PlymouthUniversity and Professional Training & Development Department To work inEducational settings, Children Centres, and clinics
Under the supervision of the practice mentor/teacher,participates in the assessment of health needs and formulation of care plansfor children, young people, their families and communities, this includes theimplementation of programmes of care, offering support and advice to parentsand families in the home, clinic, school or community setting e.g. parentingskills, public health, group work and child health clinics.
In collaboration with the public health nursing team,identifies individuals and groups within the community where collaborativeworking and advice would forward the interests of public health and promotehealthy living, in particular seeking to identify disadvantaged groups andaddress health inequity.
Offers advice and support to children, young people andadults in accordance with evidence based practice.
Under the supervision of the Practice Mentor/Teachercompletes the Family Health Needs Assessment, working with the multi-agencyteam in supporting children in need and those at risk of significant harm,including observing in case conferences and other meetings and contributing toreports where appropriate. This includes learning about the role of leadprofessional or key worker for children and their families with the mostcomplex needs.

Qualifications
* We would be interested to receive applications from experienced and newly qualified nurses- NMC level 1 registration with 120 CAT points at Level 2.
* Evidence of completion of research module.
