About The Role*
A Children with Disability Social Worker in Hounslow involves assessing the needs of disabled children and their families, developing care plans, collaborating with various professionals and agencies, ensuring safeguarding procedures are followed, and connecting families with support services and their local community. The role emphasizes a comprehensive understanding of a child's complex needs, promoting their well-being, and advocating for positive outcomes.
*About The Team You'll Be Working In*
Our Children with disabilities social work team (CWDSWT) provides support for children and young people aged 0 to 18 who have severe and profound disabilities that affect day-to-day activities.
Your young person may transition to adult social care services after they turn 18.
*About You*
As a Children's Social Worker, you will play a crucial role in safeguarding and promoting the welfare of children and young people. You will work closely with families, carers, and other professionals to assess needs, develop care plans, and provide ongoing support.
Our services will always be joined up and you will develop robust partnerships with other teams and outside agencies; to include the police, health, schools and voluntary organisations.
