Sister / Charge Nurse - District Nursing

This role involves assessing and addressing the diverse health needs of the population, including patients facing end-of-life challenges and those with long-term or complex conditions. You will work under the guidance of the Integrated Team Leader and apply your clinical expertise to make vital decisions that directly impact patient outcomes.
Your days will be dynamic and fulfilling, blending hands-on patient care with mentoring and developing nursing talent to elevate care delivery in a community setting that prioritizes compassion and professionalism.
Please note this role is with the Out of Hours (OOHs) District Nursing Service, core hours between 5pm and 8am.
Responsibilities
* Assess and address diverse health needs of the community, including end-of-life care and complex or long-term conditions.
* Deliver high-quality nursing care in a community setting and exercise clinical decision-making.
* Lead and mentor junior staff; collaborate with the Integrated Team Leader to ensure effective teamworking.
* Promote compassionate, professional care and continuously seek improvements in care delivery.
