We have a number of exciting opportunities for Band 3 Reablement Assistants to be part of our Discharge to Assess Pathway 1 Service. These are innovative, permanent posts working in our 7-day service, which supports the safe, timely discharge of patients from acute and community hospitals to home. You will be working with patients in their own homes and community bed based settings.
The team adopts a Reablement approach to help them maximise our patient’s independence and make informed choices about their long-term care needs. Our aim is to develop a culture and understanding that being in a home environment is more beneficial for individuals than being in hospital.
Main duties of the job
We are looking for flexible team players who have a background in care. The role will involve working independently and as a team member. You will need to be happy to work a shift pattern which includes early and late shifts. You will need to be confident, professional and a good communicator. The successful candidates will be working with members of a multidisciplinary team, will need to have an NVQ3 in Health and Social Care and a willingness and ability to travel around the Ipswich and East Suffolk area.
The Therapy Service has a high profile within the Trust and thrives on innovation and challenges. The service has taken a lead role in many developments, including reablement, Discharge to Assess, REACT and Frailty Assessment Base, which is helping to shape the future of patient pathways within the local area.
Please be aware that this role require full flexibility including night sits (unless exempt ).

Qualifications
Essential criteria
1. •QCF Diploma level 3 in health/social care or NVQ 3 in health/social care or equivalent qualification/experience (if the applicant does not have an NVQ 3 but are willing to undertake further studies then we will consider their application).
2. •Willingness and ability to undertake and complete Generic Worker Training Programme
3. •Valid UK Driving Licence

5. •Previous experience of care/therapy working in health/ social care/voluntary/private sectors
6. •Awareness of the needs of older people and people with disabilities.

10. •Experience of health promotion
11. •Experience of working with people suffering from dementia
Knowledge
Essential criteria
12. •Ability to understand the rationale for inter professional working and working across health, social care and voluntary care services boundaries in order to meet the patient’s individual needs.
13. •Basic knowledge of Microsoft word and e mail process
14. •Ability to travel between locations.
