About this opportunity
This is an exciting time to join our team as we look to grow and expand the Extra Care Housing Service.
As a Social Care Worker you will support the delivery of care within the 3 services, working closley with the Social Care Worker, Supervisors and the Registered Manager in planning, co-ordinating and delivering a quality service to the people we support in Westmorland & Furness.
You will oversee the development of support plans and risk assessments with a focus on achieving outcomes, ensuring support plans are followed and reviewed, maintaining accurate records (using appropriate ICT systems) to meet the needs of the people we support. Also, you will be required to work in partnership with a range of stakeholders and ensuring health and safety requirements are met.
This role will include working across 3 services Kendal, Windermere and Kirkby Stephen. You will be required to travel, sometimes at short notice, visiting people within their own flats in the different schemes.
