Our Supported living services help people to live as independently as possible. You will be responsible delivering support for people to live in a safe, excellent quality and happy home.

Your Purpose


Support the Service Coordinator in managing the day to day operations of the [supported living/respite/day] service. Support your team of frontline managers to provide safe and high quality care for individuals they support and help your team develop their skills and achieve their goals.


Your Line Manager


Your line manager will be the Service Coordinator.


Your Role

* Recruit, induct, train and manage staff to delivery high quality, person-centred care, and ensure they are up to date with best practice
* Lead a team of [House Managers/Unit Managers] to provide safe, high quality and person-centred services to individuals we support
* Make sure the right staff, with the right skills are working in the right places to best meet the needs of individuals we support.
* Support, monitor and develop your team to perform effectively in their role through regular 1:1s, coaching and mentoring
* Consult with people we support and their families in order to seek feedback on service provision and plan future improvements.
* Complete quality checks on services to make sure our services are being delivered in line with our expected standards, regulatory, legislative and contractual requirements.
* Identify areas for improvement, develop actions plans and monitor completion to ensure improvements are made
* Coordinate excellent comprehensive care with the individual, health and other external professionals, and their families/friends where required
* Monitor and maintain accurate records and documentation for all operational areas of the service
* Working within the organisation's policies and procedures, maintaining records and reports accordingly.
* Prepare performance information and management reports and contribute to service development and planning
* Undertake internal investigations in line with the relevant PossAbilities policies and procedures
* Respond to safeguarding concerns, incidents, and other concerns about service quality and take prompt action
* Provide on-call support
* Deliver training and other development opportunities for staff teams to improve their knowledge and skills
