Responsibilities
The post holder will be responsible for the assessment of individuals in the community, home, nursing homes and hospitals to determine eligibility to receive NHS Continuing Healthcare or Funding Nursing Care. The assessor will act as a member of the multidisciplinary team to coordinate the process, establish eligibility and assist in identifying a suitable care package. The assessor will be responsible to ensure the review process for all NHS Funded Care is undertaken in accordance with current guidance.
The practitioner will utilise the national assessment tools and adhere to the required timescales, coordinate, participate and support the assessment process with the multidisciplinary team. The role will be part of the MDT and determine the individual’s eligibility in line with the NHS Framework for NHS Continuing Healthcare and Funded Nursing Care. This will encompass explaining the process to patients and families; health and social care colleagues and care providers. Ensuring that the process is transparent and clearly understood by all parties.
