Overview
The role is to support patients to exercise safely and effectively after a Myocardial Infarction, Coronary Artery Bypass Surgery, Valve repair/replacement and Percutaneous Coronary Interventions in community venues. The post provides highly specialized assessment and exercise prescription for patients with complex needs, offers exercise guidance and information, and acts as a resource for other staff and health care professionals. The post leads the provision of exercise prescription and delivers the exercise component of the Cardiac Rehabilitation Program across various delivery models.
Responsibilities
* Assist in the assessment, planning, delivery, and evaluation of evidence-based exercise for patients with coronary artery disease in community venues.
* Carry out individual patient functional capacity assessments using standardized exercise assessment tools recognised by BACPR.
* Screen and assess patients, interpret clinical investigations, conduct appropriate submaximal tests to establish a baseline functional capacity, and apply this to exercise programming.
* Carry out individual mobility assessments and risk stratification.
* Interpret and evaluate functional capacity and modify individual exercise programs for comorbidities.
* Collaborate with cardiac nurses to ensure safe, effective assessment, planning, and delivery of the exercise component of the programme.
* Provide prescription of individualized exercise programmes relative to patients’ physical, psychological, and social needs.
Qualifications
Essential
* Degree in Physiotherapy / Exercise Physiology / Sports science
* HCPC Registered
* Evidence of Continuing Professional Development
* British Association of Cardiac Prevention and Rehabilitation Level 4 training
Desirable
* The Chartered Institute for the Management of Sports and Physical Activity (CIMSPA) Level 4

Experience
Essential
* Experience of working with patients with cardiac disease.
* Experience of delivering exercise to a complex patient group (circuit-based group / gym / individual).
* Experience of working within hospital and community settings.
Desirable
* Experience of data collection and analysis.
* Participation in research or audit.
* Experience in adapting exercise to those with comorbidities.
* Evaluating outcomes of cardiac rehab.
Knowledge
Essential
* Knowledge of national and local strategies relating to cardiac rehabilitation
* Knowledge of BACPR and ACPICR standards
* Knowledge of existing exercise, functional testing, and prescription guidelines
* Demonstrable knowledge of exercise physiology and impact of varied clinical pathologies
Desirable
* Knowledge of NACR and NICE cardiac rehabilitation guidelines
