We are expanding our Intermediate Care Teams, which deliver Discharge to Assess, Urgent Community Response, and Frailty Hospital at Home (FH@H), and are looking for a Community Rehabilitation Assistant to join our team.
As the Community Rehabilitation Assistant, you will support adult patients in their own homes and care homes. Your role will promote independence and improve quality of life by helping patients remain at home while being supported by a multidisciplinary team. You will collaborate with nurses and therapists to set goals and assist patients in regaining independence. Additionally, you will provide administrative support to ensure the smooth daily operation of the service. Strong communication skills and confident decision-making are essential for this role. You will be part of a motivated, caring, and compassionate team.

This is a full-time position, with service hours currently from 08:00 to 20:00, seven days a week. Being a driver is essential for this role.

The role involves providing rehab-based care in patients' homes, including short-term personal care. You will monitor deteriorating patients with clinician support. Training will be provided to achieve competency in clinical observations, venepuncture, bladder scanning, and ECG as required.
Employment requires an Enhanced Disclosure and Barring Service (DBS) check, which the Trust will cover. Applicants subscribed to the DBS update service can present a valid DBS certificate instead of a new check.
