Care Coordinator
Location: Bromborough, Wirral (travel required)
Salary: £26,000–£28,000 per annum
Working Pattern: Full-time (including on-call rota)
Driving Licence: Essential (own vehicle required)
An opportunity has arisen for a dedicated Care Coordinator to support the delivery of high-quality, person-centred care across residential and community services.
You will work with young people and adults with complex needs, including learning disabilities, autism, physical disabilities, and mental health needs. This is a fast-paced role where you will coordinate care packages, support staff teams, and ensure services meet regulatory standards.
Key Responsibilities
* Coordinate care delivery and maintain continuity of care
* Manage staff rotas, availability, and cover
* Support recruitment, onboarding, and staff development
* Monitor compliance with CQC regulations and internal policies
* Maintain care plans, risk assessments, and accurate records
* Liaise with families, professionals, and internal teams
* Participate in the out-of-hours on-call rota
Requirements
* Experience in complex care or supported living
* Level 3 Health & Social Care (or willingness to work towards)
* Knowledge of relevant legislation and CQC standards
* Strong organisational and communication skills
* Ability to work under pressure
* Full UK driving licence and access to a vehicle
