Job summary
We have a permanent vacancy for an Advanced Nurse Practitioner.
We are looking for a motivated Advanced Nurse Practitioner to deliver high standards of patient care, using clinical skills and a broad, in-depth theoretical knowledge base.You will use your experience to assess, manage and refer independently.
This is an additional role, to join our team and work alongside our existing three ANPs.
Salary negotiable depending on experience with additional Simply Health Cash Plan
30 days holiday (whole time hours) plus bank holidays
This is a part time role three days per week Wednesday/Thursday/Friday 8.30am 5pm (24 hours).
Main duties of the job
Your role will encompass surgeries, minor illness, home visits and assessing elderly patients.You will be confident managing telephone appointments and be keen to participate in clinical meetings.

Job responsibilities
The post holder will act autonomously within their professional scope of practice providing care for patients face to face, telephone consultations, care homes and home visits.
The post holder will operate within their own sphere of competence and agreed acceptable limits of practice to provide expert professional clinical advice to patients, carers and colleagues, ensuring clinical safety and excellence.
The post holder will need to prioritise and triage the needs of patients accordingly, making any necessary referrals for investigations in the appropriate manner.

* Work as part of a multidisciplinary team to deliver safe and high quality care.
* Carries out clinical nursing practice at an advanced level using expert clinical skills, critical judgements & knowledge to deliver appropriate and safe care.
* Delivery of care in line with NSF, NICE guidelines and evidence-based care
* Triage and treat patients, performing specialist assessment of their needs and make referrals as necessary to other members of the primary and secondary health care teams as appropriate.
* Co-ordinate the planning and delivery of care ensuring patients and carers/relatives are involved.
* Prescribing and reviewing medication that is appropriate to patient needs and in accordance with latest evidence-based practice, national and practice protocols and within the scope of qualified practice.
* Diagnose and manage acute and chronic conditions, including drug and non-drug based treatment methods into a management plan.
* Help care for patients in care homes, through delivery of DES and help with care of housebound in delivering high quality ongoing chronic disease management.
* Request diagnostic investigations and interpret within agreed protocols e.g. x-ray, blood tests, scans, etc.
* Assess, diagnose and treat patients in practice premises, in care homes or their own homes who require medical attention and/or refer to appropriate agency.
* Refers patients to other members of the multi-disciplinary practice teams, secondary care organisations and other organisations as necessary.
