JobSummary:
The purpose ofthis post is to provide a holistic assessment of families and offersingle or multiagency coordinated packages ofsupport.
The Caseworkerwill empower families to identify and tackle challenging behavioursreduce negative family relationships securing a positive pathwayfor improvement with particular emphasis upon increasingeducational attendance attainment and reduce exclusionsby:
1. Working in partnership withfamilies offering listening advice and guidance in order to developand enhance parenting skills.
2.Delivering evidencebased parenting programmes to a defined clientgroup.
3. Completing assessments usingthe Early Help Framework and Signs of Safety model to makeappropriate referrals to otherservices.
4. Taking on the role of theLead Professional coordinating services in the Team Around theFamily (TAF) acting as the single point of contact when required.To work in partnership with relevant professionals in statutory andnonstatutory organisations
5.Supporting families who do not meet the thresholds that trigger theinvolvement of specialistservices.
The post holderwill work in partnership with the family to identify broker andsecure appropriate services to meet identified need as well asproviding direct support to schools to embed early helpprocesses.
KeyDuties/Accountabilities:
Tocarry a complex caseload and key work with families who arereferred for support taking on the lead professional role tocoordinate a multiagency whole family approach to addressdifficulties.
Toundertake whole family assessments ensuring a high standard ofservice to families including securing the services of otheragencies.
Toreview action plans regularly and coordinate activity to ensureagreed tasks are identified and goals reached in a timelymanner.
Toensure that the service provide is delivered at times that suit andmeet the needs of the families e.g. early mornings lateevenings.
Towork in partnership with social workers to effectively stepdowncases to the Early Help Service thus reducing social carecaseloads.
To maintainaccurate and detailed case documentation files with evidence ofdecisionmaking processes and statutory safeguarding actionsrecorded in relevant ITsystems.
Follow ChildProtection and Safeguarding procedures alerting respective seniorsaccordingly to any risk issues that may arise including personalsafety and safety ofothers.
EssentialExperienceRequired:
To havesignificant experience of direct work with children young peopleand their families in the family home or in thecommunity.
Experience ofworking effectively in cooperation and partnership with a range ofcommunities partner agencies private sector bodies public agenciesvoluntary bodies and statutory agencies appropriate to the servicearea.
Working as amember of a multidisciplinary team and in partnership with parentsprofessional and voluntary sector to meet the needs of vulnerablechildren and families who have multiple and complex needs toachieve sustainable and positivechange.
Experience ofworking with complex case reviews such as multiagency professionalmeetings e.g. Multi Agency Panels Safeguarding or TAFs (teamsaround thefamily).
EssentialQualificationsRequired:
NVQ level 3qualification or equivalent in the field of Childcare EducationHealth.
