Job Title: Care Coordinator
Reports To: Operations Manager
Location: Birmingham (on-site)
Contract Type: Full-time / Permanent
Salary: £30,000 per annum, commensurate with experience and skills
Purpose of the Role:
As the Care Coordinator, you will play a vital role in organising and maintaining the delivery of high-quality, person-centred care to clients living in their own homes. You will be responsible for coordinating care staff schedules, ensuring compliance with care plans, and maintaining communication between clients, families, and the care team.
Key Responsibilities:
* Manage and coordinate rotas, ensuring all packages are fully staffed.
* Be the first point of contact for care staff – resolving queries, offering guidance, and building strong rapport.
* Ensure compliance with CQC standards and maintain up-to-date digital care records.
* Ensure care workers are fully briefed on clients’ needs and any changes in care plans.
* Respond to last-minute scheduling changes, sickness, or emergency cover needs.
* Monitor the quality-of-care delivery and assist in training and supervision of staff.
Requirements:
* Previous experience in care coordination, scheduling or similar roles.
* Working knowledge of care regulations and CQC standards.
* Strong communication, multitasking, and problem-solving skills.
* Confidence using digital care planning tools and scheduling systems.
* A team player with a passion for delivering outstanding care services.
