Overview
We are recruiting a Band 6 Memory Nurse for the Countywide EMDASS Team in West Hertfordshire. The role involves delivering memory assessment, dementia diagnosis, and post‑diagnostic support, working closely with a multidisciplinary team to enhance service efficiency. Hertfordshire Partnership University NHS Foundation Trust has achieved an overall rating of Outstanding from the Care Quality Commission.
Responsibilities
* Conduct memory assessments and deliver dementia diagnoses using appropriate assessment tools.
* Coordinate treatment and post‑diagnostic support through titration reviews.
* Work across the One‑Stop Pathway and Primary Care settings.
* Carry out risk assessments and implement risk‑management plans for service users and carers.
* Apply safeguarding policies and protocols to ensure safety in the community.
* Collaborate with partner agencies to tailor referrals and support service users in living well with their illness.
* Act as Duty Nurse for screening referrals, offering advice and signposting.
* Supervise junior staff and trainees as delegated by the MATS Team Leader.
* Support the transformation of the EMDASS service to improve pathway efficiency.
Qualifications
* Band 6 banding with appropriate nursing and memory/dementia care experience.
* Excellent organisational and communication skills.
* Strong clinical leadership and ability to support student and junior staff.
* Self‑motivated, proactive, and flexible to meet service needs.
