An exciting opportunity has arisen for a successful individual to join the Continuing Health Care Team in Swansea Bay University Health Board.
The post holder will be part of a small dynamic team based in the community. You will be expected to work over a 7 day rota with 12 hour shifts, working days and nights.

Qualifications
Essential criteria
1. Minimum of Qualification Credit Framework - QCF Level 2 (formally NVQ Level 2) or Has the equivalent knowledge, skills and experience acquired through induction and on the job training and willing to complete QCF qualification within agreed timeframe in Health
2. Willing to undertake further learning specific to role
3. Evidence of maintaining their personal development
4. Knowledge and understanding of the Code of Conduct for Healthcare Support Workers in Wales
