Practice Nurse Castle Gate Medical Practice
Practice Nurse required. Experience of treatment room duties and respiratory chronic disease management is desirable. Ideal working pattern is Tuesday, Wednesday and Friday. Salary dependant on experience.
Main duties of the job
Primarily dealing with treatment room duties and chronic disease management (respiratory, diabetes, hypertension, coronary heart disease) both via telephone and face to face.
To facilitate and participate in multidisciplinary education within the practice.

Castle Gate Medical Practice (formerly Chippenham Surgery) has been providing medical care to local patients since approximately 1840. Since then we have grown to provide care for a list size currently standing at approximately 13,700 patients. We have a branch surgery in Raglan (9 miles from Monmouth). We have 9 GPs working at our Practice in addition to a great team of other clinicians, receptionists, admin and support staff. We encourage our healthcare professionals to develop their own professional development plans, allowing individuals time to teach and study. We are a registered training Practice for Registrars.

Job responsibilities
We are seeking a motivated and committed Practice Nurse to join our supportive, enthusiastic and friendly team.
The salary is negotiable and will be dependant on qualifications and experience. The successful applicant will be required to work in both of our surgeries, Monmouth and Raglan.
We are looking to develop and increase our current Nursing team. You will be working alongside our existing Practice Nurses, HCAs and Phlebotomists to develop the service we currently offer our patients. You will be fully supported by our GPs & practice management team and as a patient-focused practice in turn you will be asked to deliver the highest quality service to our patients and our practice community. The successful applicant will be a dynamic individual, able to evidence qualifications and experience in a wide range of medical disciplines within primary care and general practice. The successful applicant will also need to evidence knowledge of QAIF and chronic disease management.
