Job Overview
We are looking for an Occupational Therapist to join the North Lancashire Integrated Community Therapy & Rehabilitation Service, working across the Royal Lancaster Infirmary and community offices. The role is part of a newly integrated Community Therapy Team supporting Home Assessment discharges, urgent and routine Community Therapy Assessment & Rehabilitation.
Responsibilities
* Manage a clinical caseload across the North Lancashire district and associated hospitals.
* Support Home Assessment discharges from hospital while on rota to fulfil the Responder role.
* Provide urgent response for same‑day/next‑day discharge, and to referrals prioritised within 3‑day, 2‑week, 4‑week and 10‑week pathways.
* Deliver falls prevention, community rehabilitation, long‑term condition management and continuing healthcare referrals to patients’ homes, residential or nursing homes across Lancaster District.
* Provide therapy at the 23‑bed step‑down residential rehabilitation unit.
* Conduct dynamic, holistic assessments at point of discharge; integrate findings, risk‑taking principles; support patients and families to overcome risks and meet OT needs.
* Make ongoing rehabilitation and social care referrals where appropriate.
* Work collaboratively with Clinical Lead Therapists, Integrated Care Communities, local GPs, Adult Social Care, Advanced Clinical Practitioners and hospital discharge teams.
* Contribute to rota for urgent response and community pathway work.
* Provide feedback and clinical supervision as required.
Qualifications
* Full UK driving licence and access to own car.
* Qualified Occupational Therapist.
* Ability to travel across south Cumbria and North Lancashire.
* Flexible for varying service needs and cross‑boundary work.
