Main duties of the job
* Triaging E consults as operate a total triage system
* Managing acute and minor illness presentations
* Long-term condition reviews (depending on experience)
* Home visits (if applicable)
* Working closely with GPs, nurses, and our admin team
* Contributing to a collaborative and innovative approach to primary care

Job responsibilities
* Independently assess, diagnose, and manage patients presenting with a wide range of conditions.
* Prescribe medications appropriately in line with current guidelines and within scope of practice.
* Manage acute illnesses, chronic disease reviews, minor injuries, and urgent care presentations.
* Order and interpret diagnostic tests, investigations, and referrals.
* Undertake health promotion, disease prevention, and screening initiatives.
* Provide expert advice and support to patients, carers, and colleagues.

* Masters degree in Advanced Clinical Practice or equivalent
* Independent Prescriber (V300 qualification)
* Proven experience working autonomously in primary care, urgent care, or similar settings
* Excellent clinical reasoning, decision-making, and communication skills
* Commitment to patient-centred care and multidisciplinary working
Desirable
* Experience using EMIS clinical IT systems
* Special interests (e.g. womens health, minor illness, frailty, mental health)
* Teaching, mentoring, or leadership experience
