Job Title: Adult Social Worker – Hospital Discharge
Location: Barnsley (Hybrid)
Pay Rate: £35 per hour
Liquid Personnel is currently recruiting for an Adult Hospital Social Worker on behalf of our client. This is an exciting opportunity to join a dedicated hospital discharge team, where you will play a vital role in supporting adults during their transition from hospital to home or other care settings.
Responsibilities:
* Experience working with health partners or hospital social work discharge teams.
* Strong social work values that guide professional practice.
* Focused on helping individuals achieve the best outcomes following hospital discharge.
* Committed to promoting independence through a strengths-based approach.
* Collaborates effectively with individuals, families, carers, and communities.
* Supports informed decision-making processes.
* Works closely with health colleagues under the hospital’s Discharge to Assess process.

Qualifications & Experience:
* Social Work England registration
* Eligible to work in the UK
* Hold a full UK driving licence
* Post-qualified experience
* Degree level or equivalent in Social Work
