Overview
Ranworth PCN is seeking an Advanced Nurse Practitioner to work within the Acute 'on the day' service. The position is offered on a full-time basis, with part-time considered. The role will be based at the Kennedy Way Medical Centre with travel to all practices within the PCN.
Responsibilities
* Assess, review and evaluate the needs of patients
* Deliver treatments or refer patients as required
* Prioritise patients within the service
* Provide necessary treatment including non-medical prescribing and/or patient group directions in line with local/national policies, procedures and guidelines
* Medicines management, leadership and case management
* Manage and act upon clinical triage between 8am and 10am each day
* Carry out duties including suturing and/or fracture management
* Maximise patients\' health and reduce risks contributing to ill health, thereby reducing unnecessary admissions to acute services
Person Specification
Qualifications
* Registered nurse (NMC)
* Experience of working within primary care and as a nurse practitioner
* Post graduate diploma or degree (advanced practice qualification)
* Qualified nurse prescriber
* Qualified triage nurse
* Possess a minor illness qualification
Experience
* Advanced clinical assessment skills
* Excellent communication skills
* Ability to perform in stressful and complex situations
* Sound clinical reasoning and decision making
