Up to 37.5 hours per week available.

We have exciting opportunities to join our Community Frailty Virtual Ward project, part of a wider initiative to improve urgent and emergency healthcare across Plymouth, the South Hams, and West Devon.
Our goal is to revolutionise healthcare for frail and elderly patients by launching a Community Frailty Virtual Ward, providing high-quality, patient-centred care at home, reducing hospital admissions and length of stay, and improving overall well-being through better care coordination.

All staff are expected to work across a 7-day service.
Main duties of the job
The Role
As an Advanced Clinical Pharmacist, you will be a key part of our healthcare delivery, playing a vital role in our multidisciplinary team covering two of the six new frailty virtual wards planned for our area.
Here Is What You Will Do
* Lead on medicines and medicines governance within the Virtual Ward.
* Conduct medicine reconciliation.
* Perform structured medication reviews, including complex polypharmacy and deprescribing.
* Prescribe and keep accurate medication records.
* Respond to medication queries from the multidisciplinary team and provide clinical support.
* Facilitate patient discharges.
* Work within integrated care teams including mental health, district nursing, GPs, and social workers.
