What you will do:
* Conduct comprehensive social care assessments and develop personalised care plans under the Care Act
* Manage and review case loads while maintaining accurate and timely records
* Execute safeguarding investigations to ensure the safety and protection of service users
* Engage in COPDOL work and manage complex 21a Challenges
* Perform Mental Capacity Assessments and make informed Best Interest decisions
* Support the AMHP rota as required, extending your working hours when necessary
* Participate in monthly supervisions, team meetings, and practice groups to foster continuous professional development
* Provide expert support within the team, driving collaboration and effective service delivery
* Work in a hybrid model, with flexibility to use a hub on Wednesdays and Thursdays and other Council options on the remaining days
