An exciting opportunity has arisen for enthusiastic and motivated Staff Nurses to join the Community nursing Care Team.
The post holders will provide and maintain high quality, effective, person-centred health care to people in their own homes. Liaise closely with Community Nursing Team Leader and their Deputy, and provide clinical support to Community Health Care Support Workers. Participate in the assessment process and maintain high quality effective nursing care in the community as part of the wider integrated care team.
Other duties will be to support case management of complex patients within the wider team. Support Multi-Disciplinary Team working in collaboration with key partners. Ensure service delivery is in line with the Core Offer and the Standard Operating Procedures of the Community Nursing Service.
Working Pattern: 7 days working including weekends and bank holidays

Main duties of the job
In collaboration with the District Nurse Team Leader and their Deputy, determine and prioritise the health needs of the caseload, co-ordinate the assessment, planning, implementation and evaluation of care programmes, reflecting the needs of individuals.
Participate in providing an equitable service for housebound patients working in partnership with the primary health care team, other members of the integrated care team and with statutory and voluntary agencies.
In collaboration with District Nurse Team Leader and their deputy, manage resources effectively and advise on skill mix requirements of the team.
Participate in care management for identified patients in order to prevent inappropriate hospital admissions and to facilitate timely discharge from secondary care.
Promote the principle of self-care and patient empowerment.
Positively participate in and promote Clinical Governance, thus ensuring the highest quality of practice is maintained within the service.
Take responsibility for personal competency, including participation in clinical supervision and updating knowledge on policies and procedures.
A willingness and ability to take charge when necessary and manage the caseload with indirect supervision from a senior.
To provide cover and support across the City dependent on service need.
