Overview
We are seeking a talented, qualified Advanced Nurse Practitioner (ANP) experienced in Primary Care to focus on our care homes and frail patients in Leicestershire. The role delivers telephone, online and face‑to‑face consultations and is part of a multidisciplinary team that includes GPs, nurses, pharmacists, physicians, healthcare assistants and other PCN roles.
Responsibilities
* Provide clinical care and examinations to frail patients and care‑home residents, including clinical administration as required.
* Conduct comprehensive frailty assessments and develop individualised care plans.
* Prescribe treatment, including medications, as a Non‑medical Prescriber.
* Contribute to Clinical Governance: report learning events, significant events, complaints and safeguarding concerns; perform regular audits to ensure compliance and introduce service improvements.
* Support the Registered Manager with compliance and performance targets.
* Deliver clinical leadership, support and guidance to care‑home teams and other healthcare professionals.
* Coordinate interdisciplinary teams and actively engage in MDT meetings.
* Act as an ambassador for the practice and maintain professional practice through continuing education and revalidation.
Qualifications
* Qualified or working towards a Masters level qualification in Advanced Practice.
* Independent Prescriber Qualification.
* Membership or eligibility to practice within the Nursing and Midwifery Council regulations.
* Experience within Primary Care/General Practice.
