The Opportunity We’re expanding our dedicated team of Care Coordinators and are looking for a compassionate, organised, and proactive individual to join us. What You’ll Do As a Care Coordinator, you’ll play a vital role in ensuring the smooth delivery of care services. Your responsibilities will include: Scheduling & Allocation: Matching the right carers to the right clients at the right time. Communication: Building strong relationships with clients, carers, families, and healthcare professionals. Record Management: Keeping all client and carer records accurate and up to date. Problem-Solving: Staying calm under pressure and resolving issues quickly and effectively. On-call service: Working outside normal working hours to maintain a detailed log of all on-call activities, as well as providing support and advice for field staff, clients and their families. The on-call rota require you to work 1 weekend a month and a minimum of 1 evening a fortnight. Please note the payments for the on call support is in addition to salary. What We’re Looking For A caring and professional attitude. Excellent communication and organisational skills. Ability to remain calm and patient under pressure. A valid driving licence and access to a car (essential). previous experience as a care coordinator in domiciliary care is desirable Flexibility to participate in the out-of-hours on-call rota (evenings and weekends).
