Care Co-Ordinator Care Coordinator – Join Our Growing Team! Location: Derbyshire Salary: Starting at £26,000 Plus additional Payment for participation in the out-of-hours emergency on call service of up to £8000 per annum (dependant on experience) Hours: Monday to Friday, 09:00–17:00 (with participation in out-of-hours on-call rota) About Us CRT Care Services, formerly known as Care Relief Team, has been delivering exceptional home care across Derbyshire since 1985. The Opportunity We’re expanding our dedicated team of Care Coordinators and are looking for a compassionate, organised, and proactive individual to join us. What You’ll Do As a Care Coordinator, you’ll play a vital role in ensuring the smooth delivery of care services. Your responsibilities will include: Scheduling & Allocation: Matching the right carers to the right clients at the right time. Communication: Building strong relationships with clients, carers, families, and healthcare professionals. Record Management: Keeping all client and carer records accurate and up to date. Problem-Solving: Staying calm under pressure and resolving issues quickly and effectively. On-call service: Working outside normal working hours to maintain a detailed log of all on-call activities, as well as providing support and advice for field staff, clients and their families. The on-call rota require you to work 1 weekend a month and a minimum of 1 evening a fortnight. Please note the payments for the on call support is in addition to salary. What We’re Looking For A caring and professional attitude. Excellent communication and organisational skills. Ability to remain calm and patient under pressure. A valid driving licence and access to a car (essential). previous experience as a care coordinator in domiciliary care is desirable Flexibility to participate in the out-of-hours on-call rota (evenings and weekends).
