We have a vacancy for a practice-based care coordinator at Ascent PCN.
We are looking for 14-16 hours per week. Rate of pay is subject to experience but starts at £12.21 per hour.
This role involves contacting patients to support them with their long-term health conditions and to help patients navigate their own healthcare. The care coordinator role combines traditional patient-facing responsibilities with proactive care coordination, making it ideal for healthcare professionals seeking career development in primary care.
Main duties of the job
* Inviting selected cohorts of patients in for routine monitoring
* Being a key contact for patients with complex and/or long-term healthcare needs
* Liaising with local care homes on a weekly basis
* Reviewing missed childhood immunisations
* Carry out audits to identify improvements and maximise income particularly IIF, Enhanced Service and claiming procedures
* Use Arden(s) and other appropriate population health dashboards and tools to help deliver the population health management projects
* Design efficient templates, protocols, alerts, and other IT tools to support safety, patient care, quality record-keeping, reporting and financial claims
* To assist people to access self-management education courses
* To provide co-ordination and navigation for people and their carers across health and social care services
* To work closely with Social Prescribing Link Workers and other primary care professionals
* To work with the GPs and other primary care professionals within the PCN to identify and manage a caseload of patients, and where required and as appropriate, refer people back to other health professionals within the PCN
* To raise awareness within the PCN of shared decision making and decision support tools; and how to identify patients who may benefit from shared decision making
* To be able to plan and respond to workload according to operational priorities

Person Specification
Qualifications
* The ideal candidate will be well organised and have good attention to detail. They will be a good listener and be able to communicate well on all levels to ensure patients and carers are able to navigate their healthcare needs.
* The successful candidate will have a proactive approach, the ability to work on your own initiative and within a team environment. They will have a good understanding of technology and computers and be computer literate. Experience with SystmOne and the reporting facility within SystmOne would be an advantage but is not essential.
