SEN Adulthood Officer Contract About us Our client is a local authority committed to supporting young people with Special Educational Needs and Disabilities (SEND) as they transition into adulthood. They are currently seeking a Transition Case Officer for a 6-month contract to assist in the development of new working principles aligned with evolving government legislation across Education, Health, and Social Care. This includes conducting person-centred assessments and ensuring that Education, Health and Care Plans (EHCPs) reflect the full range of support needs. Responsibilities Coordinate assessment requests and manage the full EHCP process Act as the first point of contact for young people and families during transition planning Manage a caseload of young people with complex needs, often in challenging or crisis situations Liaise with professionals across education, health, and social care to ensure holistic support Ensure all processes comply with statutory requirements and are accurately recorded Provide advice and guidance to families and service providers About You Are you passionate about supporting young people with SEND through critical life transitions? Do you have the knowledge and empathy to guide families through complex systems? If so, wed love to hear from you. Strong understanding of the needs of young people aged 1425 with complex SEND Knowledge of the rights of young people and their families, and the responsibilities of service providers Minimum of 5 GCSEs (Grades AC), including English, Maths, and ICT (or equivalent) Relevant NVQ Level 3 qualification or equivalent, or qualified by experience Additional Information Contract Duration: 6 months Working Hours: 35 hours per week Enhanced DBS check required Application Deadline: 20th June 2025
