Home Dialysis Clinical Nurse Specialist

An exciting opportunity has arisen for an experienced and enthusiastic renal nurse to join the Home Therapies Renal Team at Liverpool University Hospitals working across the Royal Liverpool Hospital & Aintree sites.
We are looking for a Home Therapies Renal Nurse Specialist to provide a quality service to patients who choose to have dialysis at home on either HD or PD.
We are seeking to recruit an individual with excellent communication skills, along with extensive post registration experience in renal replacement therapies, along with evidence of professional and personal development to degree level, or equivalent specialist study/experience. We are an enthusiastic and dedicated team and the role will not only impact on patient care but will be challenging and rewarding on a personal level.
Community work is required for this role therefore full driving license and access to a vehicle is essential for applicants.
Main duties of the job
The Home Dialysis Team is working to provide training and support for patients choosing to have dialysis at home. This is a growing service that aims to provide the highest standard of care for patients through on going education and support, both in hospital and at home.

Detailed job description and main responsibilities
* Provide education and training to Home Dialysis patients and their care givers on how to perform chosen Renal Replacement Therapy in the home.
* Coordinate the care of the home dialysis patients from referral to training and continuing follow up care in the community.
* Act as a resource for all medical and nursing staff regarding the care of hospitalised Peritoneal Dialysis and Home Haemodialysis patients.
* Provide training to patients referred to the Home Therapies Team.
* Act as a resource for patients and family members via phone/home visits /clinic visits on a timely basis.
* Participate in Quality Improvement Projects.
* Participates in the development, implementation and evaluation of nursing care standard policies and procedures for patient care.
* Identifies educational needs of inpatient and clinic nursing staff regarding care of dialysis patients.
* Perform other duties as assigned.
