Aberdeen City Out of Hours Nursing Band 6 Senior Community Nurse – The Senior Community Nurse works with and supports Band 5 and Band 3 colleagues delivering a person‑centred clinical nursing service, prioritising end of life care and symptom control for patients in their own homes and supporting admission avoidance. The role includes triage and management of unscheduled nursing referrals from GMED, NHS 24, Hospital at Home and other professionals, and provides support for the District Nurse with team management responsibilities.
Responsibilities
* Deliver a person‑centred clinical nursing service in patients\' homes, prioritising end of life care and symptom control.
* Support Band 5 and Band 3 colleagues and contribute to admission avoidance.
* Triage and manage unscheduled nursing referrals from GMED, NHS 24, Hospital at Home and other professionals.
* Provide support for the District Nurse with team management responsibilities.
* Contribute to workload planning and triage of referrals, and provide palliative and end of life care as required.
Qualifications
* Graduate Diploma in Integrated Community Nursing, or a relevant Postgraduate Certificate.
* Evidence of current clinical experience in a Community Nursing role at Band 6, including triaging of referrals, workload planning, and palliative/end of life care.
Details
* Hours: 1800–0000, 2 or 3 shifts within an 8‑day rota.
* Car owner or access to a car/driver and a current UK driving license is required. Car must be LEZ compliant.
* Location: Aberdeen Community Health and Care Village, White Zone, Frederick Street, Aberdeen AB24 5HY.
