Thisis an opportunity to make a real difference to patients lives by providingspecialist support to help them stop smoking tobacco during their hospitalstay and work independently with patients across the Trust. You will be basedat our office at St James's Hospital and required to work from there on adaily basis with travel to Leeds General Infirmary on a regular basis.
Weare looking for an enthusiastic individual who is passionate about healthpromotion and supporting patients to become smoke-free. Previous experiencein smoking cessation support or a health-care setting would be an advantagebut is not essential, as training will be provided.
Thisis currently a fixed term opportunity until 31st March 2026.

Main duties of the job
Youwill contribute to improving health and support patients to stop smoking anddevelop healthier behaviours. Our advisors work in a range of Trust sites sohaving access to a vehicle and being able to travel across the city on adaily basis is essential.
Youwill be responsible for the timely delivery of personalised stop smokingsupport to patients using interventions including: Nicotine ReplacementTherapy (NRT), Vapes, Carbon Monoxide Monitoring and tailored behaviouralsupport.

Job responsibilities
PRINCIPALDUTIES & AREAS OF RESPONSIBILITY
Acting as a key link between theTrust and community stop smoking providers to facilitate seamless and timelyaccess for patients to continued stop smoking support.
Be the main point of contact forexternal agencies for patients experiencing delays in their transfer betweenproviders of care.
Promptly alert the Team Leader ofany significant delays or difficulties with a patients access to continuedcare.
Participate in provider meetings toensure a seamless transition of care takes place between providers.
Promote the health and wellbeing ofpeople who use the service through offering specialist stop smoking advice,information and support.
Work with patients to change theirbehaviour to improve their health by stopping smoking (and, on occasions,support significant others e.g. partners/ parents/family members who smoke).
Required to use their persuasiveskills to encourage patients to change their behaviour (e.g. to try a quitattempt/abstain, to try stop smoking medication, to accept a referral forfurther support).
Manage own caseload of patients andresponsible for all associated tasks, including follow-up visits, andmanaging diary commitments.
Act as a smoke free championpromoting the smoke free message through their day-to-day work.
Support clinical staff to providemore effective stop smoking support for their patients, through providing upto date and evidence based information and guidance. Provide specialistadvice and support to staff, including delivering an education, training andawareness programme to help reduce harm from smoking and ensure LTHT remainsa smoke free setting.
Persuade other staff groups tosupport stop smoking activity with their patients and within theirdepartments more generally, identifying and supporting smoke-free championsacross the Trust.
Ensure robust discharge planning tomeet the needs of the patients ongoing stop smoking support, includingarranging onward referral/appointment (e.g. to a community stop smokingservice, community pharmacy), signposting (GP, self-help resources), agreeingfollow-up appointments/contacts, co-ordinating continued supply of medicationor prescription, feeding back to health care partners e.g. GP/primary care,midwife.
Deliver stop smoking assessments,specialist advice and support, including advice on stop smokingpharmacotherapy and e-cigarettes, working within recommended protocols andguidelines. This will involve face to face and virtual work and will requireworking closely with partners including Trust clinicians and pharmacycolleagues, Community Stop Smoking Service and community pharmacists.
In community settings only - supplypatients with Nicotine Replacement Therapy products for self-administration,in-line with Trust protocol.
Responsible for management of dayto day workload.
Communicate with a wide range ofhealth professionals and other staff to advise them on effective methods ofstop smoking behavioural support.
Undertake Carbon Monoxide (CO)testing for patients who engage with the service. Both at first contact withthe service and 28 days following discharge (where possible).
Work with a wide range of patientsas part of their day to day work, including those with communicationdifficulties and adapt information and support provided accordingly
Develop individually tailored stopsmoking support plans including recommending treatments based on assessedneeds and monitoring and adjusting plans as required.
Provide information to people aboutstopping smoking, minimizing harm from tobacco and health-related issues inline with best guidance and protocols. Taking into consideration the widerange of diverse needs e.g. different forms of tobacco use, English as asecond language Communicate using a range of resources that promote healthimprovement (mainly stopping smoking), adapting information and usingdifferent formats depending on both patient need (e.g. pregnant woman, adultinpatient) and for different staff groups or specialties.
Provide accurate and up to datepromotional information to patients and staff and design and produce newresources as/when required.
Provide advice and support to wardstaff about the service and to support their patients to quit or abstain fromsmoking whilst in their care.
Co-ordinate health improvement(mainly stop smoking) campaigns within the Trust and externally. This willrequire planning and collaboration with health improvement specialists andother colleagues (e.g. Public Health, Communications leads) and adapting information/messagesfor the intended target group.
Contribute to the monitoring andevaluation of the service, including the completion of the minimum data set(NHS England mandatory requirement). This will include following up patientsfollowing discharge to monitor stop smoking outcomes.
Responsibility to feed back to TeamLeader on any service improvements identified during day to day practice.
Use initiative tocommunicate/propose new or innovative ways of working to improve the deliveryof the service for patients.
Responsible for maintainingaccurate patient and staff records in line with Trust guidelines.
Responsible for respondingeffectively to referrals made to the service within stated timescales.
Plan and organise own dailyactivities, visiting a range of patients in various locations and situations.Plans will require adjusting depending on patient needs and, for example, ifa patient has been moved or is unable or unwilling to engage.
Work closely with Community StopSmoking Services and other providers ensuring a smooth hand over of patientcare where appropriate.
Ensure administrative duties arecarried out efficiently and effectively in order to meet the needs of theservice.
Attend meetings as and whenrequired, preparing agendas, and may include taking notes and preparingminutes, both formal and action plans.
To be involved in monitoring andevaluation of patient feedback, offering recommendations as part of the teamto improve the service.
To be involved in the marketing andpromotion of the service alongside other colleagues. This will include wardvisits, presenting to small groups e.g. team meetings and clinical forums, toraise the profile of the service and facilitate service improvements.
Undertake/support specific stopsmoking projects and/or development activities under the supervision of theStop Smoking Project Manager. Contribute to service evaluation requirements,questionnaires and surveys as required.
Responsible for audit within ownwork area.
Responsible for service improvementinitiatives. Collate and analyse the results from surveys and questionnairesto inform improvement work and present this in a range of formats fordifferent staff groups/audiences
Alert line manager to any issues inwork (including concerns about individuals or work in the Trust or in thecommunity) and maintain appropriate levels of confidentiality.

Responsibility to support theTrusts Smoke-Free policy at all times (and act as a Smoke-Free Champion aspart of day to day activity). This will include reporting breaches of policyto Team Leader and supporting the development of appropriate action.
Take every reasonable opportunityto maintain and update knowledge, competence and skill, with a particularfocus on any new developments for stop smoking.
Ensure CO monitors are in workingcondition reporting any issues with them as required.
Monitor supplies of CO monitoringdisposables and health promotion resources and supplies.
In community settings only -responsible for all aspects of stock control of Nicotine Replacement Therapyproducts in clinical settings eg maintaining stock, stock rotation, keepingaccurate record of stock supplied and/or returned, following NHS supply chainprocedures and adhering to Trust protocol at all times.
The ability to travel to andbetween agreed community venues to provide outpatient stop smoking support,(may require some home visits for maternity patients and lone working).
Responsible for maintaining stockof health improvement resources within own work area e.g. patient group orclinical specialty.

Qualifications
