Compassionate Case Managers are sought after to work with clients who have complex rehabilitation needs or life-changing injuries.
This role involves managing caseloads of children and/or adults with backgrounds of traumatic/acquired brain injuries, clinical negligence, birth trauma, spinal cord injuries or orthopaedic injuries.
You will hold a caseload which may involve home visits, travel to visit clients, families, solicitors and care teams, within your geographical remit.
Majority of cases will be managed from home but some may require face-to-face interactions in other locations. In these situations, you must have a UK driving license and own car for professional use.
The ideal candidate will have experience working as a Case Manager, either within an NHS Spinal Centre or in the medico-legal/case management sector.
They should also possess current registration with the HCPC, SWE or NMC and relevant qualifications such as Nurse, Occupational Therapist, Speech & Language Therapist, Social Worker, Sports Therapist or Social Worker.
Key Responsibilities:
* Manage a caseload of clients with complex rehabilitation needs or life-changing injuries
* Conduct home visits, travel to client locations, and communicate with healthcare professionals
* Evaluate client's condition and develop treatment plans to meet their needs
* Collaborate with multidisciplinary teams to provide comprehensive care
* Document progress, updates, and outcomes of case management
Contract Options:
Both Employed and Associate contracts are available, requiring a minimum commitment of 3 days per week. Salaries/Rates are dependent on experience and vary between £35,000 - £60,000 (employed) and up to £70/hour (associate).
