Job summary INTERNAL MPFT APPLICANTS ONLY An exciting opportunity has arisen for anexperienced and motivated paediatric or learning disability to join our team. The post is for 37.5 hours Term time only. The team provides holistic nursing care for pupils with a wide range of learning disabilities, physical disabilities and neurological conditions. The successful applicant will work within the Tamworth hub covering a Nursery, Primary and High school. Working as a member of a friendly, highly skilled t e am you will have the opportunity to make a difference to the lives of children and young people who have special needs and their families. You will be responsible for their healthcare needs during the school day and work in partnership with education colleagues in the school and with parents. You will also work collaboratively with the wider Multi Agency Teams to meet health, developmental and safeguarding needs of the children and young people. The post holder will be expected to work flexibly and will need to be able to transport themselves and any equipment to the different locations within the hub. There will a requirement for travel to other areas/hubs to support with cover and attendance at team meetings and training. Access to a vehicle for work purposes is essential. Main duties of the job The post holder will work as part of a team to participate in the delivery of health care and promote the health and welfare of children attending Staffordshire Special Schools and to acknowledge and respect the individual rights and personal dignity of the students. The post holder will work autonomously within their sphere of professional practice, working within teams and service guidelines and objectives. To be involved in the assessment, planning, implementation and evaluation of individual care needs in line with the core offer of service. To ensure high quality care from the Special School Nursing Service consists of: Assessment and review Hands on clinical care Key worker and signposting Excellent communication Training and supervision General role in optimising long term health and lifestyles Transition of young people to adult education and services Deputising for Team Leader as required. Coordinate the appropriate allocation of case work in relation to the hub area. This is to include appropriate safeguarding delegation. To carry out prescribed nursing interventions as per the care plan, which will identify; Existing clinical needs and how and by whom these will be met. Monitoring the childs growth and development in line with the Healthy Child Programme. Progress against the care plan, expected outcomes and timescales. The care plan will be made available to sit alongside and complement the childs individual Education Plan. To prioritise daily care, responding to emergencies as required. To assess the needs of the school age population and promote healthy lifestyles within the Public Health Role of School Nursing. To act as a lead for the Special School Nursing Service, in the promotion of high-quality care. This will include training of staff, developing pathways & representing the team at key meetings. To undertake continence assessments and liaise/refer to Specialists as required. To ensure safe custody and administration of prescribed medication where required in accordance with Trust policies and NMC guidelines. To liaise appropriately and effectively with parents/carers, education staff and other agencies to achieve optimum health in school age children and young people. To demonstrate, teach and supervise aspects of care that can be delegated to support staff and education staff. To contribute to the induction of new staff to the service. To ensure that KPI data is received by Team Leaders in a contemporaneous manner each month by specified deadline. Participate in the prevention, identification and protection of children at risk in accordance with Trust Safeguarding Procedures. To facilitate and attend school medicals and undertake nurse led assessments where relevant. To provide advice to parents, pupils and school staff on preventative measures to limit outbreaks of infectious diseases Observes own personal duty of care to equipment and resources.
