Role Overview
As a Care Coordinator in the Woodbridge Integrated Neighbourhood Team, you will help shape Suffolk’s future through innovation, collaboration, and community impact. You will support people to live well and independently at home, building strong relationships, streamlining services, and reducing unnecessary hospital admissions.
Key Responsibilities
