Adult Social Worker Stockport Council – Bridgehall Community Team Up to £30ph Stockport Council is seeking a motivated and experienced Adult Social Worker to join our dedicated community team based at Bridgehall Community Centre. This is a vital full-time, community-based position where you will be an integral part of our ‘One Stockport’ approach, promoting independence, choice, and a ‘gloriously ordinary life’ for the adults we support. Key Responsibilities As a Neighbourhood Social Worker, your key duties will include: * Safeguarding Vulnerable Adults: Leading on complex safeguarding inquiries, risk assessment, and protection plans. * Care Act Assessments: Conducting thorough and timely needs assessments under the Care Act 2014, focusing on strengths and assets. * Support Planning: Co-producing robust and personalised support plans that maximise independence and reflect individual choice and control. * Multidisciplinary Working: Building strong, effective relationships with health partners, the voluntary sector, and other council services. * Hospital Discharge: Facilitating timely and safe transitions from hospital back into the community, upholding the principles of Discharge to Assess. * Commissioning and Direct Payments: Making effective commissioning decisions and supporting individuals to manage their care via Direct Payments. * Practice Leadership: Providing support and guidance to less experienced colleagues and promoting excellent social work standards.
