Job summary

An excitingopportunity has arisen for a motivated and enthusiastic Advanced ClinicalPractitioner (ACP) to join our dynamic team at Briarwood Medical Centre.

We are seeking ahighly skilled clinician to deliver high-quality, person-centred care, workingautonomously and as part of our multi-disciplinary team. You will play a keyrole in supporting the clinical workload, including triage, assessment,diagnosis, treatment, and evaluation of patient care within your scope ofpractice.

Working closely withthe wider team and reporting to the GP Partner and Practice Manager, you willhelp to ensure delivery of safe, effective, and timely care while alsocontributing to service improvement and clinical governance. The postholderwill also support our preparations for regulatory compliance, including CQCinspection readiness.

Main duties of the job

* Undertaking patient consultations independently, including physical examinations, diagnostic decision-making, and treatment planning.
* Prescribing medications within your professional competence and scope of practice.
* Supporting long-term condition management and urgent care presentations.
* Promoting high standards of clinical care and supporting quality improvement initiatives.
* Working collaboratively with clinicians, administrative staff, and external partners to improve outcomes and service delivery.

Clinical Practice

Undertake holistic clinicalassessments, diagnoses, treatment planning and prescribing for patientspresenting with undifferentiated and undiagnosed conditions.

Manage long-term conditionssuch as hypertension, asthma, COPD, diabetes, and heart failure in accordancewith national guidelines.

Conduct medication reviews andoptimise medicines use, particularly in polypharmacy, frailty, andmultimorbidity.

Independently manage minorillnesses and acute presentations using evidence-based practice.

Perform clinical examinations,interpret diagnostic tests, and initiate appropriate referrals when needed.

Prescribe and deprescribesafely and appropriately within scope of practice.

Manage caseloads and triageappointments, home visits or telephone/video consultations where required.

MedicinesOptimisation

Lead on repeat prescribingsystems, prescription queries, and formulary compliance.

Support QOF and IIFachievement through targeted medication reviews and audits.

Identify cost-effectiveprescribing opportunities and liaise with local pharmacy teams.

Provide clinical leadershipand mentorship to other pharmacy team members (e.g., technicians, pharmacyassistants).

Education,Mentorship and Leadership

Supervise and mentor juniorclinical pharmacists and pharmacy technicians within the practice/PCN.

Support induction andupskilling of multi-disciplinary team members on medication-related topics.

Lead quality improvementprojects including audits, guidelines implementation, and clinical governanceactivities.

Collaborationand Teamwork

Work closely with GPs, nurses,paramedics, social prescribers, and other MDT members to deliver integratedcare.

Liaise with communitypharmacy, secondary care and PCN pharmacy teams to ensure seamless medicinesmanagement.

Actively participate inclinical and business meetings, including clinical governance reviews and MDTdiscussions.

* Postgraduate qualification in Advanced Clinical Practice (MSc or equivalent, Level 7)
* Independent Prescriber (V300)
Desirable

* Additional qualifications (e.g., in diabetes, respiratory, frailty)
