We are looking for a part itme Band 5community nurse who can be responsible for delivering a high standard ofnursing care to patients in a community setting.
The successfulcandidate will be able to work independently, assessing and providing care toindividuals with various health needs. This will include those with chronicillnesses, post hospitalisation care, wound management, and palliative care.This list is not exhaustive. The role involves collaborating with otherhealthcare professionals and the wider MDT, supporting patient recovery, andpromoting self-care in negotiation with patients and their carers and families.
Main duties of the job
Assess, plan, implement, and evaluate patientcare in community settings.
Provide wound management including dressings,compression therapy, and other related interventions.
Support patients with chronic diseasemanagement e.g diabetes, respiratory conditions.
Deliver End-of-life care and support patientswith palliative care needs.
Health education and self-care facilitation forpatients to promote their independence.
Conduct initial assessments and regular reviewsof patients health conditions.
Maintain accurate and up-to-date clinicalrecords.
Ensure adherence to infection control andhealth and safety protocols.
Monitor patients for any changes in theircondition, responding appropriately to emergencies and changes.
Refer patients to other healthcareprofessionals when necessary.
Work closely with GPs, physiotherapists,occupational therapists, social workers and other healthcare professionals toensure holistic care.
Provide support and advice to patientsfamilies or carers, ensuring they are involved in the care process
Attend Multidisciplinary team meetings andcontribute to care planning
Engage in continuous professional development(CPD) to keep update with the latest nursing practices and guidelines

Person Specification
Qualifications
* RN qualification on part 1 or part 12 of the register
* Post registration qualifications relevant to community nursing.
Experience
* Up to date clinical knowledge including wound care, management of continence and palliative care.
* Understanding of equality and diversity

* Basic IT skills
* Able to work independently recognising own limitations.
