Job Title: Adult Review Team Social Worker (6-month Contract)
Location: Wakefield
Salary: £35 per hour

Position Overview: We are currently seeking a motivated and dedicated Social Worker to join our Adult Review Team on a 6-month contract basis. As a member of the Adult Review Team, you will play a crucial role in conducting assessments, reviewing care plans, and coordinating services to ensure the best possible outcomes for adults in need of support.
Key Responsibilities:
1. Conducting comprehensive assessments of adults in need of care and support.
2. Reviewing and updating care plans to reflect changing needs and circumstances.
3. Coordinating with multi-disciplinary teams to deliver integrated and person-centered support services.
4. Providing advocacy and support to ensure that adults receive the services and assistance they require.
5. Liaising with external agencies, including healthcare providers and community organizations, to access additional support and resources.
6. Monitoring and reviewing the effectiveness of support plans and making adjustments as necessary.
7. Maintaining accurate and up-to-date records of assessments, interventions, and outcomes.
Qualifications and Experience:
8. Qualified Social Worker with relevant experience in adult social care.
9. Registration with Social Work England or equivalent regulatory body.
10. Knowledge of relevant legislation, policies, and procedures related to adult social care.
11. Strong assessment, communication, and interpersonal skills.
12. Ability to work collaboratively as part of a multi-disciplinary team.
13. Commitment to promoting the rights, dignity, and well-being of adults in need of support.
