PCN Social Prescriber - Walsall West One PCN

Modality PartnershipWalsall Division has a full-time vacancy fixed term contract for 12 months for a PCN Social Prescriber. The postholder will work as part of amulti-disciplinary team within general practice at Kingfisher Berkley Practicewithin Modality Walsall Primary Care Network PCN. We are seeking dedicated postholdersto join our team and who can display our CARE values: Commitment, Accountability,Respect and Excellence.

This job issuitable for individuals who are career driven and have a passion for working inan agile manner, and who are committed to make real changes to the health careservices provided within Primary Care to improve patient care. Working with our other PCN roles andgeneral practice colleagues, including health and wellbeing coaches, carecoordinators and physician associates, the postholder will provide holistic andperson-centred support.

Main duties of the job

The role of theSocial Prescribing Link Worker is to provide social support to patients acrossthe PCN. You will work individually with patients, taking the time to listenand understand what matters to them, and supporting them to access additionalservices, groups or social activities.

The postholderwill need to confidently evidence and utilise key skills such as effectivecommunication, ability to empower people to take control of their healthand wellbeing through referral to link workers who give time, focus on whatmatters to me and take a holistic approach to an individuals health andwellbeing, connecting people to diverse community groups and statutory servicesfor practical and emotional support.

You willcarry and manage a case load of patients, working independently within yourremit to support patients. All social prescribers receive clinical supervision.

The postholderwill work in collaboration with local divisions (including Divisional Boards) andthe National Modality Business Support Team to deliver innovative ways ofworking to meet the strategic aims of the organisation.

The role is nota regular 9am-5pm job, the job requires innovation, flexibility, and commitment. The postholder will be required to workresourcefully as part of the team to ensure tasks are completed.

Person Specification


Knowledge

* NVQ Level 3, Advanced level or equivalent qualifications or working towards.
* Demonstrable commitment to professional and personal development.
* Training in motivational coaching and interviewing or equivalent experience (advantageous).
* Knowledge of the personalised care approach.
* Understanding of the wider determinants of health, including social, economic and environmental factors and their impact on communities, individuals, their families and carers.
* Understanding of, and commitment to, equality, diversity and inclusion.
* Knowledge of IT systems, including ability to use word processing skills, emails and the internet to create simple plans and reports.
* Local knowledge of VCSE and community services in the locality.


Personal Qualities

* Able to work well under pressure and deliver to tight timescales.
* Well organised.
* Proficient verbal and written communication skills to individuals at all levels within and outside of the organisation
* Strong work ethic as part of a team and working on your own initiative.
* Able to respond effectively to changing priorities.

Experience

* Experience of working directly in a community development context, adult health and social care, learning support or public health/health improvement (including unpaid work).
* Experience of supporting people, their families and carers in a related role (including unpaid work).
* Experience of partnership/collaborative working and of building relationships across a variety of organisations.


Skills

* Ability to actively listen, empathise with people and provide person-centred support in a non-judgemental way.
* Able to provide a culturally sensitive service, by supporting people from all backgrounds and communities, respecting lifestyles and diversity.
* Able to support people in a way that inspires trust and confidence, motivating others to reach their potential.
* Ability to communicate effectively, both verbally and in writing, with people, their families, carers, community groups, partner agencies and stakeholders.
* Ability to identify risk and assess/manage risk when working with individuals.
* Have a strong awareness and understanding of when it is appropriate or necessary to refer people back to other health professionals/agencies, when what the person needs is beyond the scope of the link worker role e.g., when there is a mental health need requiring a qualified practitioner.
* Able to work from an asset-based approach, building on existing community and personal assets.
* Ability to maintain effective working relationships and to promote collaborative practice with all colleagues.
* Ability to organise, plan and prioritise on own initiative, including when under pressure and meeting deadlines.
* Ability to work flexibly and enthusiastically within a team or on own initiative.
* Knowledge of, and ability to work to, policies and procedures, including confidentiality, safeguarding, lone working, information governance, and health and safety.
