Community Psychiatric Nurse - County Dementia Outreach Service

Gedling & Hucknall Community Mental Health Services is an established service offering assessment and treatment for older people with mental health difficulties and for people of any age with organic diagnosis within the Gedling & Hucknall locality. The County Dementia Outreach Service operates within a “virtual team” model with clinicians based within community mental health teams. We utilise dementia friendly, non-pharmacological, person centred care approaches, working collaboratively with residents, their loved ones and care home staff to enhance wellbeing. Our remit is to reduce distress and behaviours of concern and to reduce unnecessary hospital admissions while maintaining appropriate care placements.

Role: Community Psychiatric Nurse – Band 6, with substantial dementia experience in inpatient/residential and/or community settings.


Responsibilities

* Advise and support care home staff to improve skills and confidence in managing behaviours and promoting person centred care planning.
* Apply specialist knowledge of nursing issues related to dementia in a residential setting; maintain high standards of written, verbal and IT communication.
* Demonstrate a robust understanding of risk assessment and risk management; manage a caseload effectively.
* Understand the mental and physical health and social needs of the client group; have a working understanding of Safeguarding issues and Deprivation of Liberty safeguards (DoLS).
* Show time management and organisational skills and the ability to work under pressure; act as a nursing student mentor.


Qualifications

* Experienced, NMC Registered Band 6 Community Psychiatric Nurse with substantial dementia experience in either inpatient/residential or community settings.
* Strong communication skills (verbal and written) and good IT skills.
* Ability to work within a multi-disciplinary, virtual team and to contribute to person centred care planning.
