We are looking to recruit a highly motivated, innovative and enthusiastic Neurodiversity Complex Case Lead to join our forward‑thinking, friendly and expanding Health & Justice team at HMP Send.
Responsibilities
* Lead in the identification, assessment, support of people with Neurodevelopmental Disorders, including Autism, Attention Deficit Hyperactivity Disorder, Intellectual & Developmental Disabilities.
* Provide & support others to offer adapted psychosocial interventions for people whose communication, thinking style & coping strategies are impacted upon by the presence of a ND.
* Hold a complex caseload of people who are identified to have needs associated with ND & require reasonable adjustments to be made to their healthcare.
* Keep up to date with research, policy & legislation relevant to ND.
* Offer expert consultancy to the Mental Health Team & contribute to person‑centred pathways & interventions within the MDT.
* Develop person‑centred Care Plans which identify strengths & needs in collaboration with individuals.
* Supported by your clinical lead to improve the outcomes of people with ND at HMP Send, delivering against the national specifications for primary & mental healthcare for people with ND.
* Deputise in the absence of line manager when required & play an active part in the Healthcare Management Team.
* Recognise that there is need for flexibility, working outside of normal working hours if required, including weekends.
* Maintain own skills & knowledge to a high standard & ensure that this is reflected within the team.
Additional key responsibilities include maintaining a complex caseload, keeping up to date with research, policy & legislation, offering expert consultancy, developing person‑centred care plans, deputising line manager and contributing to the Healthcare Management Team, working outside normal hours if required and maintaining high standards of skill and knowledge.
Quality of Care
* Promote the health and well‑being of patients and maintain a safe, caring, and therapeutic environment based on an individualised approach.
* Take responsibility for a caseload of clients, maintaining clinical skills.
* To support work to develop and use appropriate screening tools for Neurodevelopmental Disorder as part of an integrated care pathway.
* To develop knowledge and skills in treatment of patients with Neurodevelopmental Disorders (ASD, ADHD and IDD).
* To deliver a range of adapted psychosocial, and psycho‑education sessions for individuals with neuro‑developmental disorders and help others adapt similar programmes.
* To maintain accurate, timely paper and electronic records, ensuring they are kept safely in accordance with CNWL policies, including the Data Protection Act (1998) and the CNWL Confidentiality Policy.
* To lead on specific care quality initiatives, within the team and service‑wide.
* To participate in managerial supervision and the appraisal process.
