An exciting opportunity has arisen to lead our Early Intervention and Prevention community team working with adults 18 years and over in Telford and Wrekin CQC good rated authority. Your team will receive all new contacts to adult social care and all individuals post enablement Pathway 1. The team will deliver robust strengths based conversations and assessments based in the community hubs, independent living centre and peoples' own homes to achieve aspirational outcomes for individuals and their families. The team comprises of qualified social workers, adult practitioners and hybrid workers (trained to complete Care Act assessments and occupational therapy assessments). This fast paced busy team is crucial in delivering the council's Making Prevention Real agenda and the Team Leader will play a pivotal role in designing and delivering this new service.
