We are delighted to be supporting SCCCC, a charity very close to our heart, on their search for a Discharge, Support and Admission Prevention Worker to join their growing team in Doncaster. This position sits within the Home from Hospital team, and supports individuals when they have been discharged from hospital. This role will make an enormous difference to the lives of people in Doncaster. This people-focused role will directly support older and vulnerable people in the Doncaster community. Some service users may be lonely, and socially isolated and need a helping hand when they are being discharged from hospital. This role is part time working either 21, or 28 hours per week and is well suited to a friendly and caring individual.
Responsibilities
 * Liaising with individuals to discuss pre-discharge process.
 * Ensuring that any medicines prescribed is available in readiness for discharge.
 * Checking that the main services within the individuals home are switched on prior to discharge, including heating and water.
 * Check basic food stocks within the home.
 * Facilitate the installation of any needed equipment within the home such as bed levers and walking frames etc.
 * Ensure the safe discharge of the individual from hospital.
 * Provide follow-up support after discharge.
 * Help with signposting to various agencies as required.
 * Low level support with basic things such as food preparation and shopping.
 * Undertake a level 1 falls risk assessment with the individual once they have been discharged.
Qualifications / Skills
 * Experience working within the community.
 * Experience of working with older people.
 * Experience of risk assessments.
 * A solid understanding of the challenges that older people are faced with.
 * Excellent communication skills including in person and telephone skills.
 * Super organised.
 * Accurate record-keeping.
 * Ability to adhere to policies and procedures.
 * Car driver and access to own vehicle.
 * Clear DBS.
