Your Role
As a key member of the Hospital Team, your responsibilities will include:
* Hospital Discharge Planning: Complete Care Act assessments and arrange safe, timely discharges through the Discharge to Assess (D2A) model.
* Integrated Working: Collaborate with health professionals, therapists, housing officers, and others to coordinate discharge planning and aftercare.
* Reablement Focus: Promote independence and recovery, aiming to minimise reliance on long-term care and avoid hospital readmission.
* Safeguarding Adults: Respond to concerns in accordance with Section 42 of the Care Act and local safeguarding policies.
* Mental Capacity & Advocacy: Conduct Mental Capacity Assessments and best interest decisions; ensure access to advocacy when needed.
* Care Reviews: Monitor and evaluate care packages for effectiveness, quality, and value for money.
* Documentation: Maintain accurate records in line with legal and local authority standards using electronic case management systems.
What We’re Looking For
* Social Work degree or equivalent.
* Registered with Social Work England.
* Prior experience in adult social care, ideally in hospital discharge, intermediate care, or reablement.
* Solid understanding of the Care Act 2014, Mental Capacity Act, and NHS Discharge to Assess processes.
* Confident working in a multidisciplinary team.
* Strong assessment, planning, and time-management abilities.
* Experience with complex decision-making and working under pressure.
* Knowledge of Continuing Health Care (CHC) and NHS-funded care pathways.
* Familiarity with Mosaic or similar case management systems.
* Calm, organised, and resilient under pressure.
* Strong commitment to person-centred care.
* Positive, collaborative, and solution-oriented approach.
