Occupational Therapist - Community Mental Health

An exciting opportunity has arisen for a Band 5 Occupational Therapist to join our Community Mental Health Team in Ammanford and Towy Valley.

You will be based with our supportive and friendly community team, providing valuable support focused on the needs of the individual. You will be a key member within a multidisciplinary team supporting an integrated model of care. You will provide assessment and intervention for patients living in the community. As part of an integrated team you will work closely with acute, intermediate and primary care. In promoting health and person centred care you will develop strong links with community based resources to help engage and motivate individuals in their recovery.

This team covers the Ammanford and Towy Valley area and you will be based in Swn y Gwynt resource centre in Ammanford.

Main duties of the job

To practice as an autonomous practitioner with an Occupational therapy focussed caseload within a MDT, to plan, modify & carry out assessments, and occupationally based interventions.

To formulate and apply relevant occupational analysis to the individual, using graded occupations as an essential aspect of the assessment and treatment.

To agree goals and priorities for intervention and the methods to be adopted based on results of assessment, best practice and in partnership with the patient and multidisciplinary team.

To design and implement individual or group treatment programmers to address defined clinical needs.

To utilise group dynamics to achieve agreed therapeutic objectives.

To monitor, modify and evaluate intervention in order to measure progress and ensure effectiveness of intervention.

To advise and support patient during psychological adaptation to disability.

To provide advice and support to relatives, carers and others as appropriate.

To work independently with patients, taking responsibility for the patient's safety when undertaking OT intervention.

To make clinical judgments and priority of referrals for Occupational Therapy and on the need for referral to other services.

To carry out environmental risk assessment.

To be able to define a differential diagnosis on the basis of evidence from assessments.

* Two integrated care centres: Aberaeron and Cardigan in Ceredigion, and several other community settings
* 47 general practices (six of which are health board managed practices); dental practices (including four orthodontic); 97 community pharmacies; 43 general ophthalmic practices; and 8 ophthalmic domiciliary providers
* Numerous mental health and learning disabilities services

Person Specification


Qualifications & Knowledge

* Degree in Occupational Therapy
* Current HCPC Registration
* Knowledge of relevant current health and social care legislation applicable to role
* Knowledge and application of Microsoft Office software.
* Understanding of the roles of other staff groups in health and social care.
* Awareness of clinical governance, including research and audit methodology.
* Awareness of relevant outcome measures to demonstrate effectiveness
* Knowledge of other services that link across primary/secondary/intermediate care
* Working knowledge of therapeutic equipment
* Knowledge of Health Board and priorities.
* Knowledge of a range of rehabilitation interventions across all therapies.


Experience

* Experience of working in therapy or other relevant field, which may include practice education, voluntary or paid employment.
* Working with a variety of people with differing needs
* Managing a caseload.
* Working as part of a team.
* Direct involvement with carers and families.
* Self-development and application of learning
* Use of adapted techniques for improving patient outcomes
* Contributing to improving services
* Experience of working with groups
* Experience of working in a community setting
* Delegation and Supervision of staff
* Experience of coping independently with distressing or emotional situations in challenging work environments
* Experience of using virtual communication platforms for delivering service.

* Welsh Speaker (Level 1)
