An exciting opportunity has arisen to join the Early Intervention Community Team (EICT) based across various locations. Successful candidates will work across busy Multi-Disciplinary Teams to facilitate Social Work practices, delivering person‑centred practices, enabling citizens to exercise choice, control and independence, and building resilience.
The focus will be on prevention and early intervention, supporting, guiding, and signposting individuals, as well as carrying out assessments using the 3 Conversations model in line with the Care Act 2014.
Responsibilities:
* Work as part of the EICT Pathways to deliver person‑centred practices and support individuals.
* Carry out social work assessments and use the 3 Conversations model.
* Apply relevant legislation to conduct conversations and provide support and guidance.
* Work positively as part of a Multi-Disciplinary Team and partner with other stakeholders.
* Maintain pace, demonstrate resilience, and deliver under pressure within required timeframes.
Qualifications and experience:
* Demonstrable social work experience.
* Skills and interest to work within the specific EICT Pathway.
* Strong interpersonal and communication skills.
* Ability to work at pace and demonstrate resilience.
