The residential support worker a key leadership and operational role within Graceful Healthcare Services Limited, ensuring the consistent delivery of high-quality, person-centred care that meets CQC standards and organisational values.

Leadership

• Provide daily supervision and guidance to Resident.

• Coordinate rotas, ensuring all shifts are covered and staff are deployed effectively.

• Act as the first point of contact for resident during shifts, offering practical and professional support.

• Promote a culture of accountability, teamwork, and open communication.

• Lead by example in demonstrating empathy, professionalism, and ethical care.
Care Delivery and Oversight

• Complete hands-on support and care shifts as required to maintain service quality including personal care.

• Ensure all care is delivered according to individual care plans, risk assessments, and service user preferences.

• Conduct general assessments of care delivery, identifying areas of good practice and areas requiring improvement.

• Promote and model trauma-informed and person-centred approaches in all care interactions.

Documentation, Auditing, and Quality Assurance

• Ensure that all entries are factual, objective, and recorded in real time.

• Carry out spot checks and quality assessments across homes to maintain CQC compliance.

• Produce and submit weekly summary reports detailing observations, actions, and updates.

• Address gaps in documentation and guide staff in proper record-keeping techniques.
Communication and MDT Coordination

• Facilitate and participate in Multidisciplinary Team (MDT) meetings, ensuring collaborative planning and holistic care for each service user.

• Prepare summaries and updates for MDT meetings, ensuring all relevant information is shared.

• Liaise professionally with families, advocates, healthcare professionals, and external agencies.

• Maintain effective communication channels between staff, management, and external partners.

Compliance, Safeguarding, and Professional Standards

• Ensure all care delivery complies with CQC Fundamental Standards, Health and Safety Regulations, GDPR, and Safeguarding Policies.

• Immediately report safeguarding concerns or breaches of professional boundaries.

• Uphold confidentiality and data protection principles at all times.

• Support the Registered Manager in ensuring the service meets or exceeds CQC inspection expectations.

Reporting and Administrative Duties

• Submit timely, accurate reports to management, including incident summaries, staffing updates, and care quality observations.

• Maintain up-to-date staff supervision and appraisal records.

• Contribute to service audits and internal reviews to support continuous improvement.

• Clear understanding of organisational reporting structure

Development and Continuous Improvement

• Engage in professional development and attend all mandatory and specialist training.

• Promote reflective practice and a learning culture across the team.

• Encourage feedback from service users, families, and professionals to drive improvement initiatives.

Other specialist skills responsibilities

• The individual would be involved in the preparation of person-centred support plan and risk assessment.

• Be actively involved in patient assessment for placement

• Good understanding of medication administration, safeguarding and being able to effectively notify the necessary channels when any of the above requires urgent response.
Skills and Attributes Required

• Strong leadership, coordination, and problem-solving abilities.

• Excellent written and verbal communication skills.

• Understanding of CQC Key Lines of Enquiry (KLOEs) and care compliance.

• Compassionate, respectful, and person-centred approach to care.

• High attention to detail and commitment to accurate documentation.

• Ability to mentor, coach, and motivate diverse staff teams.

• Confidence to represent the service in MDT and professional meetings.

Qualifications and Experience

• Minimum NVQ Level 3 in Health and Social Care (or working towards).

• Minimum 2 years' experience in a care or support setting, with at least 1 year in a senior or supervisory position.

• Knowledge of the Mental Capacity Act (2005), Safeguarding, and GDPR principles.

• Experience using digital care recording systems.
